Step 1 - Review Parent/Guardian Informational Letter
Please read this Informational Letter addressed to all 8th grade parents/guardians regarding trip dates, cost, payment schedule, where to turn in payments, contact information, and academic and conduct requirements.
Step 2 - Registration
Please complete the Registration form HERE (click through link to go to registration form; it will open as a separate tab). Just because you register online does not mean you are finished! See Steps 3-5 for other documentation we need for your child to go on this trip.
IMPORTANT NOTE BEFORE MOVING ON TO STEPS 3-5:
Steps 3-5 contain forms that need to be filled out and turned in to the Hopewell front office, including a permission slip with medical and movie consent, and medical forms. Please try to bring all of these documents to turn in at the same time.

Step 3 - Sign Permission Slip/Medical and Movie Consent
This Permission Slip, Medical, and Movie Consent form must be turned in along with your $140 deposit by October 13th, to Holly Schmidt in the HMS front office OR after you have paid online. These forms can also be found in the HMS front office, if needed. If your child needs to take medication on the trip, please review the forms in Step 5 and complete one or both of them ONLY IF NECESSARY.
Step 4 - Medical Forms (Should be Turned in with Permission Slip and Deposit to the HMS Front Office)
*Important Note: Students who already have prescription medications in the school clinic are able to use their existing forms and medications. We will gather the correct forms and medications from the clinic and send them on the trip with students the day of the field trip.*

- If your child already has medication filed in the school clinic, you do not need to fill out this form again.
- This form must be submitted IF medication needs to be administered to a student while on the trip.
- The medication must be supplied by a parent/guardian to the school, whether it is prescription or non-prescription medication.
- If the form is being submitted for a PRESCRIPTION, it must be signed by a physician.
- One form must be submitted PER MEDICATION.
Student Health Services SHS-2 Form: Authorization for Students to Carry a Prescription Inhaler, Epi Pen, Insulin, or Other Approved Medication (2 pages; Click on the red link to download this form.)
- If your child already has medication filed in the school clinic, you do not need to fill out this form again.
- This form must be submitted if your child needs to carry a prescription inhaler, epi pen, insulin, or other approved medications (those prescribed for emergency purposes).
- This form must be signed by a physician if it is a prescribed medication.
- One form must be submitted PER MEDICATION.
- All medications must be in their ORIGINAL containers.
- Students may carry CERTAIN NON-PRESCRIPTION medications if the SHS-2 form is filled out, with no physician signature required. The non-prescription medications that can be carried without a physician's signature are: Acetaminophen, Antacids, Aspirin, Cough or Throat Lozenges, Ibuprofen, Midol, and Oral Antihistamines.
Step 5 - Return Signed Permission Slip, Deposit ($140), and any Necessary Medical Forms to Holly Schmidt in the HMS Front Office
The permission slip must be signed and turned in at the time a deposit is made. We accept cash, checks. and money orders (checks and money orders payable to Hopewell Middle School OR HMS). Deposits WILL NOT BE ACCEPTED without a signed permission slip by a parent or guardian.
